高齢者においても脂質代謝異常は動脈硬化症の重大な 発症リスクとなる.Honolulu 心臓病調査は 65~74 歳の 日系米人高齢男性において,コレステロール値が高いほ ど虚血性心疾患の相対リスクが中年男性と同様に上昇す ることを示している. 動脈硬化症は,1大量の脂質沈着をともなう粥状動脈 硬化,2小細動脈肥厚と内腔狭窄を来す細小動脈硬化, 3筋型動脈の中膜に石灰沈着を来す Moenckeberg(メ ンケベルグ)型中膜石灰化硬化に分類される.このうち 粥状動脈硬化の初期病変は動脈内皮表面の小斑点状の脂 肪沈着で,脂肪線条(Fatty Streak)と呼ばれる.脂質 線条は 10 代のヒト動脈でも見られ,病変が年余にわた り進むことを示す.脂肪線条はコレステリルエステルを 蓄積した主にマクロファージからなる泡沫細胞の集簇で あり,粥状動脈硬化の成立にコレステロール代謝が深く 関わる...
